For Appointments with Serene Telehealth, referrals must be addressed to Serene Telehealth. Please review our CONSULT FEES before sending us a referral.
Obtain a G.P Referral
EMAIL THE REFERRAL
FAX THE REFERRAL
REGISTRATION AND APPOINTMENT PROCESS
If your registration form is not received within 14 days, we will send the referral back to your GP.
If any assessments or scripts are issued you will be sent an email. Assessments must be completed prior to your next appointment.
If any fees are outstanding, future appointments will be cancelled. If you have another appt, the date will sent with your invoice.
